目的观察肥胖T2DM患者血清铁蛋白(SF)、同型半胱氨酸(Hcy)水平的变化及其相关因素。方法选取新诊断T2DM患者62例和体检者38名,根据BMI分为T2DM合并肥胖(T2DM+Ob)组26例、单纯T2DM组36例、单纯肥胖(Ob)组18例及正常对照(NC)组20名,比较各组血清SF、Hcy、FPG、FIns、TC、TG、FFA及胰岛素抵抗指数(HOMA-IR)水平,并分析其相关性。结果 SF和Hcy水平在NC组、Ob组、T2DM+Ob组和Ob组逐渐升高[SF:(168.70±20.00)vs(202.11±23.48)vs(236.08±32.38)vs(286.19±34.92)ng/ml;Hcy:(6.99±1.37)vs(8.61±1.98)vs(9.97±1.30)vs(13.64±1.92)μmol/L,P<0.05]。多元逐步回归分析显示,T2DM患者SF与BMI、FFA呈正相关(P<0.05)。结论 T2DM患者SF和Hcy水平高于非糖尿病人群,其中,肥胖者SF和Hcy高于非肥胖者,提示SF和Hcy可能与肥胖、糖尿病相关。
Objective To observe the changes of serum ferritin (SF) and homocysteine (Hcy) in obese T2DM patients and its related factors. Methods Totally 62 newly diagnosed T2DM patients and 38 healthy subjects were divided into two groups: T2DM + Ob group (26 cases), T2DM group (36 cases), Ob group (18 cases) and normal control group (NC) The serum levels of SF, Hcy, FPG, FIns, TC, TG, FFA and insulin resistance index (HOMA-IR) in each group were compared and analyzed. Results The levels of SF and Hcy increased gradually in NC group, Ob group, T2DM + Ob group and Ob group [SF: (168.70 ± 20.00) vs (202.11 ± 23.48) vs (236.08 ± 32.38) vs (286.19 ± 34.92) ng / ml; Hcy: (6.99 ± 1.37) vs (8.61 ± 1.98) vs (9.97 ± 1.30) vs (13.64 ± 1.92) μmol / L, P <0.05]. Multiple stepwise regression analysis showed that there was a positive correlation between SF and BMI and FFA in T2DM patients (P <0.05). Conclusion The levels of SF and Hcy in T2DM patients are higher than those in non-diabetic patients. SF and Hcy in obese people are higher than those in non-obese subjects, suggesting that SF and Hcy may be related to obesity and diabetes.
