女,14岁,因右股部蔓状血管瘤第14次接受平阳霉素注射治疗。注射方法:在胸透及血象检查正常情况下,取平阳霉素8mg溶解于注射用水2ml,抽0.2ml皮内注射,观察半小时无不良反应后,将已溶解的平阳霉素注入瘤体内每隔14天注射一次。本次用药1分钟后出现口舌和全身麻木,双眼失明,旋即口唇及全身皮肤颜色发绀,畏寒,恶心、呕吐胃内容物2次,腹痛。查体:P56次/分、R18次/分、Bp13/9kPa。意识清楚,全身皮肤青紫。双瞳孔等大等圆、直径2mm,眼前指动,光感消失。双肺、腹部正常。神经系统:病理反射未引出。立即给予非那根25mg肌注,5％葡萄糖500ml+地塞米松10mg静滴,6分钟后视力恢复正常,但球结膜充血明显,全身肤色微红,余症状缓解。30分钟后,球结膜充血明显减轻,全身肤色、恢复正常。1小时后,除精神欠佳外,余均恢复正常。
