上一讲我们主要介绍了出院后糖尿病人社区管理基本流程的概况,本讲我们将进一步对基本流程进行详细的解释和解读。上一讲中我们指出,根据出院后糖尿病复查血糖结果的不同,可以把病人分成2大类,一类病人是出院后复查的血糖仍然达标,另一类病人是出院后复查的血糖没有达标。对这两类不同病人的具体社区管理方法如下。
The last talk we mainly introduced the basic process of community management of discharged diabetic patients overview, this talk we will further explain the basic process and interpretation. In the last talk, we pointed out that according to the different post-discharge diabetes mellitus, the patients can be divided into two categories. One type of patients is still discharged after the review of blood glucose is still standard, the other type of patients is discharged after discharge of blood glucose did not meet the standards. The specific community management methods for these two different types of patients are as follows.
