目的:探索鼻咽癌(NPC)放射治疗的眼部并发症。方法:对114例NPC首程放疗患者进行跟踪观察,最长35个月,最短6个月。于放疗前作视力、裂隙灯、电脑视野及视诱发电位检查。对发生角膜上皮损伤者进行角膜追踪观察。结果:NPC首程放疗患者114例,发生角膜上皮损伤5例,占4.4%。Ⅰ、Ⅱ期患者3例,Ⅲ、Ⅳ期各1例。5例患者发病时及观察期间均无明显畏光、流泪及疼痛等症状。放疗后10个月,角膜病变基本消退,但知觉仍较对侧眼明显减退。结论:NPC放疗导致角膜上皮损伤,无明显畏光、流泪及疼痛等自觉症状,可能与角膜感觉神经损害有关。放疗导致角膜损伤与NPC临床分期似乎无相关关系,可能与不同个体对辐射敏感性的差异有关。
Objective: To explore the ocular complications of nasopharyngeal carcinoma (NPC) radiotherapy. Methods: 114 patients with NPC first-line radiotherapy patients were followed up, the longest 35 months, the shortest 6 months. Prior to radiotherapy for visual acuity, slit lamp, computer vision and visual evoked potential test. Corneal epithelial injury occurred corneal tracking. Results: In the first NPC of NPC, 114 cases were treated with corneal epithelial injury, accounting for 4.4%. Three patients were in stage Ⅰ and Ⅱ, one in stage Ⅲ and Ⅳ. There were no symptoms of photophobia, tearing and pain in 5 patients during onset and during observation. 10 months after radiotherapy, corneal lesions subsided, but the perception is still significantly lower than the contralateral eye. Conclusion: NPC radiotherapy leads to corneal epithelial injury, no obvious photophobia, tearing and pain and other symptoms, which may be related to corneal sensory nerve damage. Corneal injury caused by radiotherapy does not appear to be related to the clinical stage of NPC, and may be related to the differences in radiosensitivity among different individuals.
