脊索瘤是一罕见的恶性肿瘤,发生在脊柱轴线上,一般认为由脊索组织发生。本病发生于神经和驱干骨交界处附近,且多适宜手术切除原发瘤。放疗多用于术后有可见的肿瘤残留或术后复发。一般认为放疗收效不大且放疗的最佳剂量与分割亦无定论。尽管可发生远区转移,但本病患者多死于局部未控。本文报告了Margaret 公立医院对脊索瘤病人使用中等剂量常规分割的超高压放疗,也报告了几例用超分割放疗的疗效。本组结果与其它用超高压放疗的文献报告作了比较,对一般认为脊索瘤只能用高剂量放疗的观点提出质疑。
Chordoma is a rare malignancy that occurs on the axis of the spine and is thought to occur by spinal cord tissue. The disease occurs near the junction of the nerve and the dried bone, and is suitable for surgical resection of the primary tumor. Radiotherapy is mostly used for postoperative residual tumors or postoperative recurrence. It is generally believed that radiotherapy is not effective and the best dose and segmentation of radiotherapy are inconclusive. Although distant metastases may occur, patients with this disease die more than local control. This article reports on the use of moderate-dose, routinely-segmented hypertensive radiotherapy for patients with chordoma in the Margaret Public Hospital. Several cases of hyperfractionated radiotherapy have also been reported. The results of this group were compared with other literature reports using ultra-high pressure radiotherapy and questioned the notion that chordoma can only be treated with high-dose radiotherapy.
