目的探讨雾化使用痰热清注射液对慢性阻塞性肺疾病(COPD)急性加重期痰热阻肺证患者体内炎症因子、肺功能指标的影响。方法将100例COPD急性加重期痰热阻肺证患者分为治疗组和对照组各50例,均给予抗感染、舒张气道、祛痰等常规治疗,治疗组加用痰热清注射液雾化吸入治疗。比较治疗前、后2组患者血浆TNF-α、IL-6、IL-10等炎症因子水平,白细胞总数及中性粒细胞总数、肺功能指标变化。结果 12组患者治疗后血浆TNF-α、IL-6、IL-10因子水平较治疗前显著降低(P<0.05),治疗组患者TNF-α、IL-6指标较对照组比较降低(P<0.05)。22组患者治疗后白细胞总数及中性粒细胞百分比水平较治疗前降低(P<0.05),治疗组较对照组明显降低(P<0.05)。32组患者治疗后肺功能通气指标FEV1.0/FVC%、MMEF等明显上升(P<0.05)。4治疗组和对照组总有效率分别为92.0%和76.0%(P<0.05)。结论痰热清注射液雾化使用可有效阻止COPD急性加重期患者体内炎症因子释放,减轻气道炎症损伤,促进痰液排出、减轻呼吸困难等而改善通气的功能,对控制病情有重要意义。
Objective To investigate the effect of using Tanreqing Injection on the inflammatory factors and pulmonary function indexes in patients with phlegm-heat-obstructing lung in acute exacerbation of chronic obstructive pulmonary disease (COPD). Methods 100 cases of acute exacerbation of COPD were divided into treatment group and control group, 50 cases in each group were given anti-infective, diastolic airway, expectorant and other conventional treatment, the treatment group with Tanreqing injection mist Inhalation therapy. The levels of TNF-α, IL-6, IL-10 and other inflammatory factors, the total number of leukocytes, the total number of neutrophils and the changes of pulmonary function indexes in two groups before and after treatment were compared. Results The plasma levels of TNF-α, IL-6 and IL-10 in the 12 groups were significantly lower than those before treatment (P <0.05), and the levels of TNF-α and IL-6 in the treatment group were lower than those in the control group 0.05). The total number of leukocytes and the percentage of neutrophils after treatment in 22 patients were lower than those before treatment (P <0.05), and the treatment group was significantly lower than the control group (P <0.05). After treatment, the FEV1.0 / FVC% and MMEF of 32 groups were significantly increased (P <0.05). 4 The total effective rate of treatment group and control group were 92.0% and 76.0% (P <0.05). Conclusion Tanreqing injection can effectively prevent the release of inflammatory cytokines in patients with acute exacerbation of COPD, reduce the damage of airway inflammation, promote the discharge of sputum, relieve the respiratory problems and improve ventilation. It is important to control the disease.
