目的 探讨纤维乳管镜诊治乳头溢液的价值。方法 对 1998年 11月~ 1999年4月 5 2例行纤维乳管检查的乳头溢液病人的临床资料进行回顾性分析。结果 对 5 2例乳头溢液病人的 6 2根乳管进行了纤维乳管镜检查 ,发现乳管肿瘤 14例 ,其中 4例为恶性 ,10例乳头状瘤 (病 ) ,乳管肿瘤诊断准确性为 10 0 % ,微小病变 (<5mm )占 85 7% ;乳管扩张症或乳管炎症 6例 ;32例乳管扩张但管腔正常。非肿瘤病人经纤维乳管镜检查冲洗后 ,2 8例病人的乳头溢液停止 ,总有效率为 73 7% (2 8/ 38)。结论 纤维乳管镜作为一种微型内镜 ,具有操作简便 ,创伤小 ,能发现乳管内微小病变 ,可能将成为替代X线乳管造影诊断乳头溢液的新方法
Objective To investigate the value of fiberoptic ductoscopy in diagnosis and treatment of nipple discharge. Methods Retrospective analysis was performed on the clinical data of 51 patients with nipple discharge who underwent fiberoptic duct examination between November 1998 and April 1999. RESULTS: Fiberoptic ductoscopy was performed on 62 breast tubes from 52 patients with nipple discharge. Fourteen cases were found to be lactated, 4 of which were malignant, and 10 were papilloma (disease). The diagnosis of ductal tumors was accurate. The sexuality was 100%, minor lesions (<5mm) accounted for 85 7%; breast dilatation or duct inflammation in 6 cases; 32 cases with dilated but normal lumen. After non-tumor patients were washed with fiberoptic ductoscopy, the nipple discharge stopped in 28 patients, and the total effective rate was 737% (28/38). Conclusion As a miniature endoscope, fiberoptic ductoscopy has the advantages of easy operation, less trauma, and can detect microscopic lesions in the milk duct. It may become a new method to replace the diagnosis of nipple discharge with X-ray angiography.
