目的探讨高密度脂蛋白胆固醇(HDL-C)在代谢综合征(MS)患者中的诊断价值及检测方法。方法选取245例MS患者(MS组)及85例健康体检者(对照组)为研究对象,测定各研究对象的空腹血糖(FPG)、血脂水平。其中采用选择性抑制法(PPD)和过氧化氢酶清除法(CAT)检测MS患者的HDL-C水平。结果采用PPD法与CAT法检验MS患者HDL-C的水平分别为(1.00±0.25)、(1.01±0.23)mmol/L,差异无统计学意义(P>0.05),两种方法之间Kappa值为0.976。MS组的FPG、胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、LDL-C/HDL-C分别为(7.22±1.89)mmol/L、(4.43±0.98)mmol/L、(2.31±0.98)mmol/L、(2.99±0.82)mmol/L、3.21±1.05,显高于对照组的(4.73±0.56)mmol/L、(4.05±0.57)mmol/L、(0.94±0.28)mmol/L、(2.52±0.65)mmol/L、0.93±0.64,MS组HDL-C为(1.00±0.25)mmol/L,低于对照组的(1.35±0.28)mmol/L,差异均有统计学意义(P<0.05)。在MS组中,FPG≥6.1 mmol/L的患者HDL-C水平[(0.99±0.15)mmol/L]低于FPG<6.1mmol/L的[(1.10±0.21)]mmol/L,TG≥2.25 mmol/L的患者HDL-C的水平[(0.98±0.24)mmol/L]低于TG<2.25mmol/L的患者,差异有统计学意义(P<0.05)。相关性检验显示,HDL-C与FPG、TG呈负相关(r分别为-0.162、-0.373,P<0.05)。结论 HDL-C在MS患者中显著降低,且随着FPG、TG的上升有下降趋势,应加强对MS患者HDL-C的监测,PPD法与CAT法均是检测HDL-C的有效方法。
Objective To investigate the diagnostic value and detection of high density lipoprotein cholesterol (HDL-C) in patients with metabolic syndrome (MS). Methods A total of 245 patients with MS (MS group) and 85 healthy subjects (control group) were selected as study objects. Fasting blood glucose (FPG) and blood lipid levels were measured in all subjects. HDL-C levels were measured in patients with MS using selective inhibition (PPD) and catalase (CAT). Results The levels of HDL-C in MS patients by PPD and CAT were (1.00 ± 0.25) and (1.01 ± 0.23) mmol / L respectively, with no significant difference (P> 0.05). The Kappa values Is 0.976. The levels of FPG, TC, TG, LDL-C and LDL-C / HDL-C in MS group were (7.22 ± 1.89) mmol / L and (2.31 ± 0.98) mmol / L, (2.99 ± 0.82) mmol / L and 3.21 ± 1.05 respectively, which was significantly higher than that of the control group (4.73 ± 0.56 mmol / L, 4.05 ± 0.57 mmol / L , (0.94 ± 0.28) mmol / L, (2.52 ± 0.65) mmol / L and 0.93 ± 0.64, respectively. The HDL-C level in MS group was (1.00 ± 0.25) mmol / L which was significantly lower than that in control group (1.35 ± 0.28 mmol / L, the differences were statistically significant (P <0.05). In the MS group, HDL-C levels in patients with FPG> 6.1 mmol / L [(0.99 ± 0.15) mmol / L] were lower than [1.10 ± 0.21] mmol / L with FPGs <6.1 mmol / The level of HDL-C in patients with mmol / L [(0.98 ± 0.24) mmol / L] was lower than that in patients with TG <2.25 mmol / L, the difference was statistically significant (P <0.05). Correlation tests showed that HDL-C was negatively correlated with FPG and TG (r = -0.162, -0.373, P <0.05, respectively). Conclusions HDL-C is significantly decreased in patients with MS. HDL-C monitoring should be strengthened with the increase of FPG and TG. PPD and CAT are both effective methods for detecting HDL-C.
