患者男,56岁,已婚,农民.因阴囊内肿块逐渐增大56年,反复右下腹疼痛2年于1984年2月5日入院.无阴囊周期性胀痛和血尿.24岁结婚,性生活正常,生有2男2女,均健在.家庭成员中无类似病史.检查:中等身材,八字胡子,喉节明显.阴囊增大,其内可触及一梨形肿块,约7cm×5cm×3cm,其蒂沿右侧外环向上延伸,质中,活动,无触痛.肿块两侧可触及3.5cm×2cm×2cm 睾丸样椭圆形肿块各1个,质中,活动,无压痛.但与梨形肿块相连,难以回纳腹腔.阴囊左、右两侧均未触及精索.透光试验阴性.肛检未发现异常.血、尿常规均正常.拟诊右腹股沟难复性斜疝.按右腹股沟斜疝手术,打开疝囊,见有一从上而下逐渐增粗的子宫颈,子宫悬吊于阴囊内,宫颈于内环下方折向盆腔.内环一指半,疝内容物已回纳腹腔,腹壁下动脉在其内侧.高位横断宫颈,连同子宫、附件一并切除,按腹股沟斜疝施行高位结扎加修补术.术后
