本文对 2 7例颅内动脉瘤活检组织进行了光学显微镜及电子显微镜观察。形态学研究证实 ,大多数动脉瘤发生在动脉分叉部位。动脉瘤壁的内皮细胞损伤、弹力板不完整或缺失以及平滑肌细胞坏死是动脉瘤破裂的形态学基础。瘤壁主要被纤维细胞及胶原所代替并出现肌纤维母细胞及前胶原物质 ,巨噬细胞增多且含有大量次级溶酶体及脂质颗粒。瘤壁全层均可见散在的淋巴细胞和单核细胞浸润 ,提示管壁的破坏可能与免疫反应有关。本文结合超微形态学所见对动脉瘤的形成及影响其破裂的因素进行了讨论。
In this paper, 27 cases of intracranial aneurysm biopsies were observed by light microscopy and electron microscopy. Morphological studies confirm that the majority of aneurysms occur at the arterial bifurcation. Aneurysmal wall endothelial cell damage, incomplete or missing elastic plate and smooth muscle cell necrosis is the morphological basis of aneurysm rupture. Tumor wall is mainly replaced by fibroblasts and collagen and myofibroblasts and procollagen, macrophages increased and contains a large number of secondary lysosomes and lipid particles. The whole wall of tumor wall can be seen scattered lymphocytes and mononuclear cells infiltration, suggesting that the destruction of the wall may be related to the immune response. In this paper, combined with the ultrastructure of the aneurysm formation and the factors that affect the rupture are discussed.
