目的 报告国内第一例由Arthrographiskalrae引起的真菌性全眼炎伴发侵袭性上颌窦、筛窦窦炎病例 ,并对眼内脓液中分离株进行鉴定。方法 从一例眼外伤患者左眼眶内清除组织及脓液中培养分离出真菌 ,并通过形态学、生化学包括尿素酶试验、放线菌酮耐受试验、最高生长温度测试、ID32生化鉴定板、药敏试验、组织病理学、辅酶Q分析和DNA序列分析等方法对该菌进行鉴定。结果 该菌形态学特征为具有双相型特点的菌落 ,镜下为典型链状关节孢子 ;生长温度最高可耐受4 2℃高温 ;对两性霉素B和伊曲康唑敏感。所含辅酶Q为辅酶Q 10 (H2 )。D1/D2和ITS区域的DNA序列分析 ,经正反链验证 ,分别确定了 6 0 4个和 5 0 8个碱基位点。通过日本国际核酸序列数据库 (DDBJ)进行同源性序列搜索证明 ,分离株与模式株UAMH36 16 (Arthrographiskalrae )的D1/D2和ITS区域有 10 0 %的碱基同源序列 ,证实为Arthrographiskalrae。 结论 这是国内首次鉴定Arthrographiskalrae,也是首次发现由该菌引起的真菌性全眼炎伴发侵袭性上颌窦、筛窦窦炎病例。
Objective To report the first case of fungal total ophthalmia caused by Arthrographiskalra in China with cases of invasive maxillary sinuses and ethmoid sinusitis and to identify the isolates in the intrapulmonary pus. Methods One case of ocular trauma patients were isolated from the left orbital tissue and pus culture fungi, and through the morphology, biochemistry, including urease test, cycloheximide tolerance test, the maximum growth temperature test, ID32 biochemical identification plate, Drug sensitivity test, histopathology, coenzyme Q analysis and DNA sequence analysis of the bacteria were identified. Results The morphology of the bacterium was a biphase-type colony with typical chain-like sporozoites microscopically. The growth temperature was up to 42 ℃ and sensitive to amphotericin B and itraconazole. Coenzyme Q contained coenzyme Q 10 (H2). DNA sequence analysis of D1 / D2 and ITS regions confirmed by forward and reverse sequences, respectively, identified 640 and 580 base pairs. Sequence homology search by Japan International Nucleotide Sequence Database (DDBJ) showed that the homology between the isolates and the model strain UAMH36 16 in D1 / D2 and ITS region was 100%, which was confirmed to be Arthrographiskalrae. Conclusion This is the first identification of Arthrographiskalrae in China. It is also the first case of fungal total ophthalmia associated with invasive maxillary sinus and sinuses sinusitis caused by this bacterium.
