目的:探讨氨基末端B型脑钠肽(NT-pro BNP)与室间隔缺损患儿早期心血管功能异常的关系。方法:选取我院心血管内科收治的先天性心脏病(室间隔缺损)患儿98例,依据小儿心衰ROSS评分分为心衰组41例(评分≥3分)和非心衰组57例(<3分),选取同期进行儿童保健的健康儿童40例作为健康组,检测并比较各组的心功能指标及相关实验室指标。结果:心衰组患儿血浆lg BNP、lg NT-pro BNP水平高于非心衰组患儿和健康组儿童(P<0.05),非心衰组患儿血浆lg BNP、lg NT-pro BNP水平高于健康组儿童(P<0.05)。心衰组患儿的LVEF、LVSF、LVCO低于非心衰组患儿和健康组儿童(P<0.05),LVEDDI、Tie指数高于非心衰组患儿和健康组儿童(P<0.05);非心衰组患儿的Tie指数高于健康组儿童(P<0.05)。lg NT-pro BNP水平与LVEF、LVSF、LVCO呈负相关(P<0.05),lg NT-pro BNP水平与LVEDDI、Tie指数呈正相关(P<0.05)。结论:NT-pro BNP与室间隔缺损患儿早期心血管功能异常具有显著的相关性,是评估室间隔缺损患儿心衰的可靠指标。
Objective: To investigate the relationship between NT-proBNP and early cardiovascular dysfunction in children with ventricular septal defect. Methods: A total of 98 children with congenital heart disease (VSD) were enrolled in our hospital. According to ROSS scores of heart failure in children, 41 cases (score≥3) and 57 non-heart failure patients (<3 points). Forty healthy children with child health care during the same period were selected as healthy group, and their cardiac function indexes and related laboratory indexes were detected and compared. Results: The levels of plasma lg BNP and lg NT-pro BNP in children with heart failure were significantly higher than those in non-heart failure children and healthy children (P <0.05). The levels of plasma lg BNP, lg NT-pro BNP The level was higher than healthy children (P <0.05). The LVEF, LVSF and LVCO in children with heart failure were significantly lower than those in non-heart failure children and healthy children (P <0.05). LVEDDI and Tie index were higher in children with HF than those in healthy children (P <0.05) Tie index in non-heart failure group was higher than that in healthy group (P <0.05). The level of NT-pro BNP was negatively correlated with LVEF, LVSF and LVCO (P <0.05). The level of lg NT-pro BNP was positively correlated with LVEDDI and Tie index (P <0.05). Conclusion: NT-pro BNP is significantly associated with early cardiovascular dysfunction in children with ventricular septal defect and is a reliable indicator of heart failure in children with ventricular septal defect.
