传染性单核细胞增多症(IM)好发于青少年,40岁以上少见。作者自1979年6月~1980年6月在匹兹堡大学卫生中心医院发现5例老年患者,其中:例1有非典型淋巴细胞增多逾2周,伴中度颈淋巴结与肝、脾肿大。骨髓内淋巴细胞占40%,见1个大的小梁旁结节。嗜异试验与Epstein-Barr病毒(EBV)滴度均提示为IM。本例不发热、无咽炎,GOT正常(<25u)。症状、体征于1月内消失;4周与6周时复查骨髓已正常。例2有发热、非渗出性咽炎。周围血见许多非典型(不成熟)淋巴细胞;颈淋巴结较大。骨髓活检示增生正常,但淋巴细胞与组织细胞集结成许多小肉芽肿。血及骨髓培养阴性;血清GOT与Ig正常。本例嗜
Infectious mononucleosis (IM) occurs in adolescents, more than 40 years of age rare. The authors identified 5 elderly patients at the University of Pittsburgh Health Center Hospital from June 1979 to June 1980, of which: case 1 had atypical lymphocytosis more than 2 weeks with moderate cervical lymph nodes and enlarged liver and spleen. Bone marrow lymphocytes account for 40%, see a large trabecular nodules. Hemophilia and Epstein-Barr virus (EBV) titers are all suggestive of IM. This case is not fever, no pharyngitis, GOT normal (<25u). Symptoms, signs disappeared in January; 4 weeks and 6 weeks to review the bone marrow has been normal. Case 2 has fever, non-exudative pharyngitis. Peripheral blood see many atypical (immature) lymphocytes; cervical lymph nodes larger. Bone marrow biopsy showed normal proliferation, but lymphocytes and tissue cells assembled into many small granulomas. Blood and bone marrow culture negative; serum GOT and Ig normal.
