目的探讨三维适形放疗联合替吉奥治疗局部晚期食管癌的疗效及毒副作用。方法选取2013年10月至2015年12月间山东省菏泽市立医院收治的80例局部晚期食管癌患者,采用信封法随机分为观察组和对照组,每组40例,所有患者均给予适形放疗。同时,观察组患者放疗第一天口服替吉奥胶囊40mg,每天两次,d1~14,共2个周期,对照组在放疗第一天行顺铂和5氟尿嘧啶(DF方案)化疗。结果观察组患者完成同期放化疗率为95.0%,对照组患者为87.5%。观察组患者的近期有效率为75.0%,对照组患者70.0%,差异无统计学意义(均P>0.05)。观察组患者3~4级骨髓抑制和恶心呕吐的发生率较对照组低,差异有统计学意义(P<0.05)。观察组患者1年和2年生存率为74.1%和54.8%对照组患者为73.3%和53.5%,差异无统计学意义(χ~2=3.91,P>0.05;χ~2=3.98,P>0.05)。结论适形放疗联合替吉奥同期化疗疗效较好,且患者依从性较好,不良反应较低,远期疗效好,值得临床推广应用。
Objective To investigate the curative effect and side effects of three-dimensional conformal radiotherapy combined with tegaserod in the treatment of locally advanced esophageal cancer. Methods Eighty patients with locally advanced esophageal cancer who were treated in Heze Municipal Hospital of Shandong Province between October 2013 and December 2015 were enrolled in this study. They were randomly divided into observation group and control group by envelope method, 40 cases in each group. All patients were given conformal Radiotherapy. In the meantime, the observation group received oral administration of Teglitazone 40mg twice a day on d1-14 for 2 days on the first day of radiotherapy. The control group received cisplatin and 5-fluorouracil (DF) chemotherapy on the first day of radiotherapy. Results The rate of chemoradiotherapy and chemotherapy was 95.0% in the observation group and 87.5% in the control group. The effective rate in the observation group was 75.0% and in the control group was 70.0%, the difference was not statistically significant (all P> 0.05). The incidence of grade 3 and 4 myelosuppression and nausea and vomiting in the observation group was lower than that in the control group, with significant difference (P <0.05). The 1-year and 2-year survival rates were 74.1% in the observation group and 54.8% in the control group, with 73.3% and 53.5% in the control group, with no significant difference (χ ~ 2 = 3.91, P> 0.05; 0.05). Conclusions Conformal radiotherapy combined with tirofiban in the same period of chemotherapy has a good curative effect, and the patients have better compliance, lower adverse reactions, long-term curative effect and worthy of clinical application.
