本文应用 Swan—Ganz 导管技术对19例慢性充血性心力衰竭病人进行了多巴酚丁胺急性血流动力学效应观察。剂量以1μg/kg/min,2μg/kg/min……输入,直到心率增快30次/分或有显著心律失常出现为止。1μg/kg/min 时即可产生有益的血流动力学效应。各个不同剂量均可使心排血量和左室作功能力增加,使肺楔压和外周阻力下降,且心指数随剂量增加而增加。4μg/kg/min 时,14例窦性心律患者心律几乎不增快,同时可显著改善各项血流动力学指标。继续加大剂量,一些重要的血流动力学指标不再改善,但心率明显加快。因而,4μg/kg/min 可能是窦性心律心力衰竭患者较合适的剂量,即可显著改善血流动力学指标,又不明显增加心率。
In this paper, 19 patients with chronic congestive heart failure were treated with Swan-Ganz catheter technique to observe the effects of dobutamine on acute hemodynamics. Dosage at 1μg / kg / min, 2μg / kg / min ...... until the heart rate increased 30 beats / min or significant arrhythmia appears so far. 1μg / kg / min can produce beneficial hemodynamic effects. Different doses can make cardiac output and left ventricular function increased, the wedge pressure and peripheral resistance decreased, and the cardiac index increased with dose. At 4 μg / kg / min, the heart rate of 14 patients with sinus rhythm did not increase significantly, meanwhile, the hemodynamic parameters were significantly improved. Continue to increase the dose, some important indicators of hemodynamic no longer improve, but significantly accelerated heart rate. Thus, 4μg / kg / min may be more appropriate dose of sinus rhythm heart failure patients, can significantly improve hemodynamic indicators, but not significantly increased heart rate.
