目的观察类风湿关节炎(rheumatoid arthritis,RA)患者血脂的变化及与病情活动指标的相关性。方法选取30例活动期RA患者与20例正常对照组,同时对受试者血清总胆固醇(total cholesterol,TC)、甘油三酯(triglyceride,TG)、高密度脂蛋白(High density lipoprotein,HDLC)、载脂蛋白A(apolipoprotein A1,ApoA1)、低密度脂蛋白(Low densitylipoprotein,LDLC)、载脂蛋白B(apolipoprotein B,ApoB)及活动性指标:类风湿因子(rheumatoid factor,RF)、红细胞沉降率(简称血沉,erythrocyte sedimentation rate,ESR)、C-反应蛋白(C-reactive protein,CRP)进行检测。结果与正常对照组相比,RA活动组TC、HDLC、ApoA1显著降低(P<0.01),ApoB显著增高(P<0.01),而TG、LDLC无统计学意义;HDLC、ApoA1与RF、CRP、ESR呈明显负相关(P<0.05或P<0.01);TC与CRP、ESR呈明显负相关(P<0.05或P<0.01),TG、LDLC、ApoB与疾病活动性无明显相关。结论活动期RA患者出现了TC、HDLC、ApoA1显著降低,表明活动期RA患者出现了的血脂变化,而这种变化与疾病的活动性密切相关。
Objective To observe the changes of blood lipids in patients with rheumatoid arthritis (RA) and their correlations with the index of disease activity. Methods Thirty patients with active RA and 20 normal controls were enrolled in this study. Serum total cholesterol (TC), triglyceride (TG), high density lipoprotein (HDLC) , Apolipoprotein A1 (ApoA1), low density lipoprotein (LDLC), apolipoprotein B (ApoB) and activity index: rheumatoid factor (RF), erythrocyte sedimentation Erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) were measured. Results Compared with the normal control group, the levels of TC, HDLC and ApoA1 in RA group were significantly lower (P <0.01) and ApoB was significantly higher (P <0.01) ESR was negatively correlated (P <0.05 or P <0.01); TC was negatively correlated with CRP and ESR (P <0.05 or P <0.01), while TG, LDLC and ApoB had no significant correlation with disease activity. Conclusion TC, HDLC and ApoA1 are significantly decreased in patients with active RA, indicating that there is a change in blood lipids in patients with active RA, which is closely related to the activity of the disease.
