目的探讨周围型肺癌CT灌注成像特点及肺癌组织病理学类型、大小和T分期对CT灌注值的影响。方法 2008年10月-2010年10月经组织病理学证实的周围型肺癌64例行螺旋CT灌注扫描,测定病灶灌注值(perfusion)、强化峰值(peakenhancement,PE)、对比剂到达峰值时间(time to peak,TTP)及血容量(blood volume,BV)等参数,并分析周围型肺癌组织类型、大小及分期等对CT灌注特征的影响。结果小细胞癌、腺癌及鳞癌的perfusion、PE、TTP及BV值比较,差异均无统计学意义(P>0.05);≤30mm的肺癌perfusion、PE及BV明显高于>30mm的肺癌(P<0.05),而>30mm的肺癌TTP较≤30mm者长(P<0.05);perfusion、PE及BV值T1期均明显高于T2期,而TTP值T1期明显低于T2期,两组间比较差异具有统计学意义(P<0.05);T3期perfusion、PE及BV值明显高于T2期,两组间比较差异具有统计学意义(P<0.05),TTP在两组间无明显差别(P>0.05);肺癌T1与T3、T1与T4、T2与T4及T3与T4期各组间CT灌注值比较,差异均无显著差别(P>0.05)。结论螺旋CT灌注成像可显示周围型肺癌微循环血流动力学改变,肺癌的组织病理学类型、大小和分期影响肺癌的CT灌注值。
Objective To investigate the characteristics of CT perfusion imaging in peripheral lung cancer and the effects of histopathological type, size and T stage on CT perfusion in lung cancer. Methods Sixty-four patients with peripheral lung cancer confirmed by histopathology from October 2008 to October 2010 underwent spiral CT perfusion imaging. The perfusion, PE, PE, (TTP), blood volume (BV) and other parameters, and analyzed the influence of the type, size and stage of peripheral lung cancer on CT perfusion characteristics. Results There was no significant difference in perfusion, PE, TTP and BV between SCC, adenocarcinoma and squamous cell carcinoma (P> 0.05). The perfusion, PE and BV of lung cancer ≤30mm were significantly higher than those of> 30mm (P <0.05). However, the TTP of lung cancer> 30mm was longer than that of ≤30mm (P <0.05). The perfusion, PE and BV values of T1 were significantly higher than those of T2, but the TTP values of T1 were significantly lower than those of T2 The difference between the two groups was statistically significant (P <0.05). The perfusion, PE and BV in T3 were significantly higher than those in T2 (P <0.05). There was no significant difference between the two groups (P> 0.05). There was no significant difference in CT perfusion between T1 and T3, T1 and T4, T2 and T4, T3 and T4 of lung cancer (P> 0.05). Conclusion Spiral CT perfusion imaging can show the micro-circulation hemodynamic changes in peripheral lung cancer. The histopathological types, size and stage of lung cancer affect the CT perfusion value of lung cancer.
