目的探讨慢性阻塞性肺疾病呼吸衰竭的治疗。方法对20例慢性阻塞性肺疾病进行常规治疗。包括抗感染、平喘、应用支气管扩张剂、使用激素及呼吸兴奋剂,纠正电解质紊乱和营养支持治疗。治疗组使用BiPAP无创通气治疗。对照组鼻导管吸氧,吸氧量1~2L/min。结果两组临床指标比较,两组治疗前后比较,各指标均有显著改变,差异有显著性。(Ρ<0.05)。4h后治疗组PaO2、pH明显增加,PaCO2、心率和呼吸频率均下降,差异有显著性(Ρ<0.05),24h后进一步改善。结论呼吸衰竭患者因管腔阻塞,气道的高反应,呼吸增快,引起呼吸肌疲劳,BiPAP具有呼吸末正压和压力支持通气作用,可降低吸气肌负荷,减少呼吸功能消耗,有利于呼吸肌休息。
Objective To investigate the treatment of respiratory failure in chronic obstructive pulmonary disease. Methods Twenty patients with chronic obstructive pulmonary disease were treated routinely. Including anti-infective, antiasthmatic, bronchodilators, hormonal and respiratory stimulants, electrolyte disturbances and nutritional support. The treatment group used BiPAP noninvasive ventilation. The control group nasal catheter oxygen, oxygen 1 ~ 2L / min. Results Comparison of clinical indicators between the two groups, before and after treatment in both groups, the indicators were significantly changed, the difference was significant. (P <0.05). After 4 hours, the PaO2 and pH of the treatment group were significantly increased, PaCO2, heart rate and respiratory rate were decreased, the difference was significant (P <0.05), and further improved after 24h. Conclusions BiPAP possesses positive end-expiratory pressure and pressure-supporting ventilation due to obstruction of the lumen, high airway reaction and rapid respiration, resulting in respiratory muscle fatigue. It can reduce the inspiratory muscle load and reduce the consumption of respiratory function, which is in favor of Respiratory muscles to rest.
