目的:探讨耻骨上前列腺切除术后大出血的原因和治疗方法。方法:回顾性分析近12 a耻骨上前列腺切除术后发生大出血的16例临床资料。结果:全部病例均于治疗后1~2 d内控制了出血。3~7 d后痊愈出院。结论:术中要细致操作,术后严密观察,排除不利因素。术后大出血者,予止血、输血、补液抗休克,调整尿管牵引方向,清除膀胱内血凝块,保持膀胱持续冲洗通畅等治疗。出血量大且难控制者,手术止血。
Objective: To investigate the causes and treatment of major bleeding after suprapubic prostatectomy. Methods: A retrospective analysis of 16 cases of massive bleeding after suprapubic prostatectomy in the past 12 years was performed. Results: All cases were controlled for bleeding within 1 to 2 days after treatment. 3 ~ 7 d after healed and discharged. Conclusion: Intraoperative should be meticulous operation, close observation after surgery, excluding adverse factors. Postoperative bleeding, to stop bleeding, transfusion, rehydration anti-shock, adjust the catheter traction direction, clear the blood clots within the bladder, keep the bladder continuous flushing and other treatment. Large amount of bleeding and difficult to control, surgery to stop bleeding.
