目的分析甲状腺超声及甲状腺功能在老年甲状腺结节的诊断价值。方法 120例甲状腺结节老年患者作为研究对象,患者均进行甲状腺超声诊断和功能检测,通过病理结果分为良性组与恶性组,观察比较两组超声诊断与功能检测结果的差异。结果经手术病理确诊,120例中良性结节79例,恶性结节41例,均为单发。超声检查显示,恶性组不规则形态、边界模糊、不均匀回声、钙化、淋巴肿大、Ⅲ级血流占比均较良性组高,而恶性组Ⅰ级血流占比(34.15%)明显低于良性组(56.96%),差异均具有统计学意义(P<0.05)。良性组血清总三碘甲状腺原氨酸(TT_3)、总甲状腺素(TT_4)、促甲状腺激素(TSH)、抗甲状腺球蛋白抗体(TGA)、抗甲状腺过氧化物酶抗体(TPO-Ab)指标水平均较恶性组高,差异均具有统计学意义(P<0.05)。TSH水平≤1.00 m IU/L,良性结节19个、恶性结节12个;1.01~4.00 m IU/L,良性49个、恶性29个;>4.00 m IU/L,良性11个、恶性0个;恶性甲状腺结节患病率不随TSH升高而增加。结论超声检查及甲状腺功能检测均能有效诊断老年患者结节病变情况,结合二者可有效提高良恶性结节诊断率,具临床推广价值。
Objective To analyze the diagnostic value of thyroid ultrasound and thyroid function in senile thyroid nodules. Methods A total of 120 elderly patients with thyroid nodules were enrolled in this study. All patients underwent thyroid ultrasound diagnosis and functional testing. The pathological results were divided into benign and malignant groups. The differences between the two groups in the diagnostic and functional tests were compared. Conclusion Both ultrasound and thyroid function tests can effectively diagnose the sarcoidosis in elderly patients. Combining the two methods can effectively improve the diagnostic rate of benign and malignant nodules and has clinical value of popularization.
