目的探讨脑钠钛(BNP)、同型半胱氨酸(Hcy)、胱抑素C(CysC)联合检测在糖尿病肾病早期诊断中的实用价值。方法收集2012年11月-2014年11月本院就诊的糖尿病患者85例,根据24 h尿白蛋白排泄率(UAER)将患者分为3组:极微量白蛋白尿组(UAER<30 mg/d);微量白蛋白尿组[UAER(30~300)mg/d]和临床蛋白尿组(UAER≥300 mg/d)。另收集门诊健康体检者40例为健康对照组,所有对象均检测BNP、Hcy、CysC及UAER,并进行分析。结果糖尿病组与健康对照组比较,BNP、Hcy、CysC明显升高,差异有统计学意义(P<0.05);糖尿病肾病的严重程度与3项指标值呈正相关(r值分别为0.732、0.725和0683,P<0.01)。结论脑钠钛、同型半胱氨酸、胱抑素C在糖尿病肾病的发生、发展中存在一定的作用,检测结果可为糖尿病肾病的早期诊断提供依据。
Objective To investigate the value of combined detection of brain natriuretic peptide (BNP), homocysteine (Hcy) and cystatin C (CysC) in the early diagnosis of diabetic nephropathy. Methods Totally 85 diabetic patients were collected from November 2012 to November 2014 in our hospital. The patients were divided into 3 groups based on 24-hour urinary albumin excretion rate (UAER): microalbuminuria group (UAER <30 mg / d); microalbuminuria group [UAER (30 ~ 300) mg / d], and clinical proteinuria group (UAER≥300 mg / d). Another 40 healthy subjects were collected as healthy control group, all subjects were detected BNP, Hcy, CysC and UAER, and analyzed. Results The levels of BNP, Hcy and CysC in diabetic group were significantly higher than those in healthy control group (P <0.05). The severity of diabetic nephropathy was positively correlated with three indexes (r = 0.732, 0.725 and 0683, P <0.01). Conclusion Brain Natriuretic Peptide, Homocysteine and Cystatin C play roles in the occurrence and development of diabetic nephropathy. The detection results may provide evidence for the early diagnosis of diabetic nephropathy.
