目的 了解无锡市阿尔茨海默病(Alzheimer’s disease,AD)患病率和家属照料者心理健康状况,并分析其影响因素,为社区制定AD的早期干预措施及提高照料者心理健康水平提供参考依据.方法 采用分层整群随机抽样方法,对无锡市4195名60岁及以上老年人进行AD筛查和临床诊断,同时对诊断为AD的居家患者家属照料者进行心理状况调查.计算并比较不同人口学特征AD患病率及家属照料者心理问题的阳性检出率,分析其影响因素.结果 无锡市60岁及以上老年人AD患病率为4.89%,居家患者的家属照料者心理问题阳性检出率为55.06%.Logistic回归分析显示,低文化程度、非在婚状态、负性生活事件是AD的危险因素;男性、相对低龄、参加娱乐活动是AD的保护因素.影响家属照料者心理健康的危险因素为被照顾患者男性、伴有精神行为症状、照料者处于在岗状态、与患者配偶关系;保护因素为被照顾患者有多个子女(≥3个)、参加职工医保、照料者家庭支持良好、每日照顾患者<6 h.结论 无锡市老年人AD患病率较高,家属照料者心理问题存在较普遍,社区应在培养老年人健康的生活方式、提高对AD患者及照料者的家庭和社会支持等方面加强关注和投入.“,”Objective To understand the status and influencing factors of Alzheimer’s disease (AD) and the psychological health of family caregivers in Wuxi and to provide evidence for the intervention of AD and to im prove the mental health of caregivers.Methods A stratified cluster sampling method was used to randomly select total 4 195 residents aged 60 years and over for the screening of AD.Meanwhile,a questionnaire survey was conducted among family member who took care of patients with AD for the assessment of their mental health.Results The incidence of AD among the elderly in Wuxi was 4.89 %,and 55.06 % of family caregiv ers were found of having psychological problems.Multivariate logistic regression analysis showed that poor education,single,having negative life events were risk factors of AD,while,male,relatively younger age,entertaining activities were the protective factors of AD.The risk factors of psychological health of family caregivers were found to be male patients,patients with behavioral and psychological symptoms,caregivers having formal jobs,patient’s spouse,while,the protective factors were patients having more than three children,having health insurance provided by their employers,strong family support for caregivers,less than 6 houvs of daily care to patients.Conclusions The incidence of AD among the elderly in Wuxi is at a relative high level and half family caregivers have psychological problems.The community should focus on cultivating healthy lifestyles among the elderly,improving family and social support for AD patients and their family caregivers.
