目的观察改良封闭胃残端方法对晚期贲门癌手术的疗效。方法断胃时尽可能保留胃大弯侧长度,将其反折与小弯侧对合封闭胃残端,然后在残胃最高处开口与食管吻合。结果本组手术效果较满意,避免了全胃切除,术后患者生活质量有所改善;术后并发症发生率较传统手术无明显变化。结论在贲门癌手术治疗中利用改良封闭胃残端方法,可有效增加残胃体积,减少死腔和吻合口张力,并避免全胃切除。
Objective To observe the curative effect of modified closed gastric stump on advanced cardia cancer surgery. Methods When the stomach off as much as possible to retain the large curved side of the length of the bend, the fold and the lesser curvature of the closure of gastric stump, and then the highest in the residual stomach open esophageal anastomosis. Results The results of this operation were satisfactory, avoiding total gastrectomy and improving the quality of life of postoperative patients. The incidence of postoperative complications was not significantly different from that of conventional surgery. Conclusion In the surgical treatment of gastric cardia by modified closed gastric stump method can effectively increase the residual gastric volume, reduce the dead space and anastomosis tension, and to avoid total gastrectomy.
