患儿女,5岁。因发热、皮疹4天,呕吐腹痛2天于1994年11月14日收住杭州市第一人民医院。4天前患儿开始发热,体温38℃(口温),继而躯干皮肤出现皮疹、水疱,伴搔痒。肌注聚肌胞及口服威乐星2天后皮疹渐结痂,但患儿出现脐周阵发性疼痛,伴非喷射性。区吐多次,口服吗丁啉无效。人院当天患儿烦躁不安,小便量明显减少。体检:T38.5℃(口温),P155次/分,R46次/分,BP10/8kPa,神志清,精神软,重病容。面色苍白,四肢凉。躯干部可见较多皮痂,背部有一新鲜疱疹。颈软。两肺呼吸音清,未闻及干湿罗音。心界无明显扩大,心率155次/分,律齐,闻及奔马律,第一心音明显低钝。腹软,无明显压痛,肝肋下2cm,脾肋下未及。肠
