目的通过开展社区公共卫生服务工作,探讨社区医院对慢性病患者的管理作用。方法选择该社区已建立健康档案的慢性病患者305人,由政府支持,3年坚持免费为慢性病患者全面体检,系统评估和干预,连续随访,健康促进。结果 2009~2012年3年来各项体检指标在逐年降低至达标。具有统计学意义(P<0.01)。结论开展社区公共卫生服务对社区医院管理慢性病有积极作用。减轻大医院就诊压力,有效利用社区医疗资源,对慢性病管理有一定效果。
Objective To explore community hospital management of chronic diseases through community public health service. Methods A total of 305 chronic patients with health records were selected from the community. The government supported the three-year free medical examination of patients with chronic diseases, systematic assessment and intervention, continuous follow-up and health promotion. Results 2009 to 2012 3 years the various physical examination indicators decreased year by year to reach the standard. With statistical significance (P <0.01). Conclusion Community-based public health services have a positive impact on the management of chronic diseases in community hospitals. Reduce the pressure of large hospitals, effective use of community medical resources, the management of chronic diseases have a certain effect.
