目的探讨牙周基础治疗对冠心病伴慢性牙周炎患者血清及龈沟液中白细胞介素(IL)-8、IL-10水平的影响。方法选取2013年2月至2014年11月在辽宁省人民医院心内科及口腔科就诊患者90例,分为3组:选取冠心病伴慢性牙周炎患者65例,随机分为两组,A组35例行心内科治疗和牙周基础治疗,B组30例仅行心内科治疗;另选无系统性疾病的单纯慢性牙周炎患者25例作为C组,行牙周基础治疗。比较各组治疗前后血清及龈沟液中IL-8、IL-10及各项牙周指标的变化。结果牙周基础治疗3个月后,A组和C组进行治疗前后自身比较,各项牙周指标均得到有效控制(P<0.05),血清及龈沟液中的IL-8降低,IL-10升高(P<0.05);A组和B组进行治疗前后组间比较,治疗前两组各项指标差异均无统计学意义,治疗3个月后A组各项牙周指标均优于B组,差异有统计学意义(P<0.05);A组血清及龈沟液中IL-8较B组明显降低,IL-10较B组明显升高,差异有统计学意义(P<0.05)。结论牙周基础治疗不仅能有效改善慢性牙周炎患者牙周局部健康情况,且能降低全身IL-8水平,升高IL-10,控制炎症感染的因素,可能是降低冠心病发生、发展的有效手段之一。
Objective To investigate the effects of basic periodontal therapy on serum interleukin-8 (IL-8) and interleukin-10 (IL-10) in patients with chronic periodontitis and coronary heart disease. Methods Totally 90 patients in Department of Cardiology and Stomatology of People’s Hospital of Liaoning Province from February 2013 to November 2014 were divided into 3 groups: 65 patients with coronary heart disease and chronic periodontitis were randomly divided into two groups: A Group 35 were treated with cardiology and periodontal basic therapy. Group B only 30 patients were treated with cardiology only. Twenty-five patients with simple chronic periodontitis without systemic diseases were selected as group C and treated with periodontal treatment. The changes of IL-8, IL-10 in the serum and gingival crevicular fluid before and after each treatment were compared with those of other periodontal indexes. Results After 3 months of periodontal treatment, the periodontal indexes were effectively controlled (P <0.05) in both groups A and C before and after treatment. IL-8 in serum and gingival crevicular fluid decreased, IL- 10 (P <0.05). There was no significant difference between the two groups before and after treatment in groups A and B, and all the indexes in group A were better than those in group A (P <0.05). The level of IL-8 in serum and gingival crevicular fluid of group A was significantly lower than that of group B, IL-10 was significantly higher than that of group B (P <0.05), the difference was statistically significant ). Conclusion The basic periodontal treatment can not only effectively improve the periodontal local health of patients with chronic periodontitis, but also reduce the level of systemic IL-8, increase the level of IL-10, and control the inflammatory factors, which may be the result of reducing the occurrence and development of coronary heart disease One of the effective means.
