目的:提高对彩超引导下经皮穿刺冷循环微波凝固治疗肝浅表肝癌的认识。方法:回顾分析5例肝浅表肝癌在彩超引导下经皮徒手穿刺冷循环微波治疗肝癌的情况。结果:5例肝浅表肝癌患者均于微波治疗术后进行了跟踪随访,4例彩超、CT复查病灶区内未见血流信号及强化,1例术后一个月彩超、CT复查病灶区内可见少许血流信号及强化,进行第二次治疗,其后彩超、CT示病灶区内亦未见血流信号及强化。现患者均健在,生存期在8~20个月之间,所有患者仍在随访之中。结论:彩超引导下经皮穿刺冷循环微波凝固治疗肝浅表肝癌直接将微波天线置入病灶动脉血供丰富的区域,阻断、破坏其血供;采用水循环内冷技术,弥补了以往微波治疗中微波天线杆温较高所带来的不足。
OBJECTIVE: To improve the understanding of the treatment of superficial hepatic superficial liver cancer with percutaneous puncture and microwave coagulation guided by ultrasound. Methods: Retrospective analysis of 5 cases of superficial hepatic liver cancer under the guidance of ultrasound-guided percutaneous cold circulation microwave treatment of liver cancer. Results: All the 5 cases of superficial hepatic carcinoma were followed up after microwave treatment. No blood flow signal and enhancement were found in 4 cases of ultrasonography and CT examination. One case was diagnosed by color Doppler ultrasound one month after operation, Visible a little blood flow signals and enhanced for the second treatment, then color Doppler ultrasound, CT showed lesion area no blood flow signals and enhanced. Patients are alive, survival in 8 to 20 months, all patients are still under follow-up. CONCLUSION: Microwave-guided percutaneous radiofrequency thermocoagulation (CLC) in the treatment of superficial hepatic carcinoma of the liver leads directly to placement of the microwave antenna into the arterial blood supply area, blocking and destroying the blood supply of the liver.
