目的:探讨垂体微腺瘤在磁共振动态增强扫描不同时期的显示特点。材料和方法:对34例微腺瘤进行Gd-DTPA增强动态扫描,在动态扫描前和扫描后作了常规SE序列扫描。结果:测量16例17个微腺瘤及邻近正常垂体的信号强度,两者的最高对比度多出现在动态增强序列105秒时。将微腺瘤与正常垂体在不同时期的对比度归纳成5种曲线,可反映微腺瘤的血供及病理改变。本组34例35个病变中,动态增强扫描的前105秒显示了34个病变,而平扫及常规SE序列增强扫描分别只显示了20个及25个病变。结论:动态增强扫描,特别是序列早期,可明显提高微腺瘤诊断的敏感性。因此,当疑为垂体微腺瘤而平扫阴性时应常规采用动态增强扫描。
Objective: To investigate the display characteristics of pituitary microadenomas in different stages of magnetic resonance imaging. Materials and Methods: Gd-DTPA-enhanced dynamic scan was performed on 34 cases of microadenomas, and conventional SE sequences were scanned before and after the dynamic scan. RESULTS: The signal intensities of 16 microadenomas and adjacent normal pituitary were measured in 16 cases, and the highest contrast of most of them appeared at 105 seconds in the dynamic enhancement sequence. The contrast of microadenoma and normal pituitary in different periods was summarized into five curves, which can reflect the blood supply and pathological changes of microadenomas. Of the 35 lesions in this group, 34 lesions were displayed in the first 105 seconds of the dynamic enhanced scan, whereas only 20 and 25 lesions were shown in the plain scan and conventional SE scans, respectively. Conclusion: Dynamic contrast-enhanced scanning, especially early in the sequence, can significantly improve the sensitivity of microadenomas. Therefore, dynamic contrast-enhanced scans should be routinely used when negativity is suspected in pituitary microadenomas.
