目的通过实时剪切波弹性成像技术对伴再生结节的肝炎肝硬化患者的肝脏进行弹性测量,探讨其在不同严重程度的肝硬化评定中的临床价值。方法选取我院诊断的肝炎肝硬化患者63例,每例患者行常规超声检查后,分成三组。第一组为线阵及凸阵探头扫查肝脏均未发现结节;第二组为线阵探头扫查肝脏发现结节,但结节直径不超过0.5cm,凸阵探头扫查肝脏未发现结节;第三组为及凸阵探头扫查肝脏均发现结节。利用SWE技术获得每组每例患者肝脏的弹性模量值,三组患者肝脏的弹性模量值进行统计学分析。结果第一组肝脏的弹性模量平均值为(10.218±1.877)kPa,第二组肝脏的弹性模量平均值为(12.760±1.945)kPa,第三组肝脏弹性模量平均值为(15.740±2.575)kPa,三组均值不来自同一总体,组间具有可比性。每两组之间肝脏弹性模量的平均值相比有统计学意义(P<0.05)。结论应用SWE技术可以区分伴再生结节的肝炎肝硬化的硬度,为临床医师评价肝硬化的严重程度提供客观依据。
Objective To measure the elasticity of hepatic cirrhosis patients with regenerative nodules by real-time shear wave elastography and discuss its clinical value in the assessment of cirrhosis with different severity. Methods Sixty-three patients with hepatitis cirrhosis diagnosed in our hospital were selected. Each patient underwent routine ultrasound examination and divided into three groups. The first group for the linear array and convex array probe scanning the liver were not found in the nodule; the second group for the linear probe scanning the liver to find nodules, but the nodule diameter of not more than 0.5cm, convex array probe scanning the liver was not found Nodules; the third group and the convex array probe scan the liver were found nodules. The SWE technique was used to obtain the liver elastic modulus values of each patient in each group and the liver elastic modulus values of the three groups were statistically analyzed. Results The average elastic modulus of the first group of liver was (10.218 ± 1.877) kPa, the second group of liver elasticity modulus (12.760 ± 1.945) kPa, the third group of liver elastic modulus was (15.740 ± 2.575) kPa, the average value of the three groups does not come from the same overall, comparable between groups. The mean value of the elastic modulus of liver between the two groups was statistically significant (P <0.05). Conclusion The application of SWE technique can distinguish the hardness of liver cirrhosis with regenerative nodules and provide an objective basis for clinicians to evaluate the severity of liver cirrhosis.
