目的研究高氟水源对妇女体内性激素结合蛋白(SHBG)与雌二醇(E2)水平的影响。方法结合水质调查结果精确监测水氟含量,将调查点选在某县的6个村庄,分别将2个饮水正常村庄、2个行降氟改水工程的村庄、2个地方性氟病村设为对照组、改水组与试验组,通过整群抽样方法选出在各村居住至少5年或当地生长的成年女性,分别纳入各组,以访问形式调查人口学特征等,采集其空腹静脉血、晨尿与当地饮用水,测定尿氟饮用水氟含量、SHBG和E2水平,并进行对比。结果三组饮用水氟含量和调查对象的尿氟含量、血清SHBG和E2水平的组间比较差异均具有统计学意义(P<0.05);试验组的水氟水平为(2.45±1.02)mg/L,高于改水组的(0.35±0.16)mg/L与对照组的(0.36±0.14)mg/L(P<0.05);对照组的尿氟含量为(0.95±0.32)mg/L,低于改水组(1.44±0.58)mg/L的与试验组的2.57±0.96)mg/L,改水组尿氟含量低于试验组(P<0.05);试验组的血清SHBG为(41.62±7.85)nmol/L,低于对照组的(57.20±8.65)nmol/L(P<0.05);试验组的血清E2水平为(52.28±11.45)nmol/L,低于对照组的(74.98±13.54)nmol/L和改水组的(75.58±14.35)nmol/L(P<0.05);试验组调查对象的血清E2水平和年龄之间存在负相关(P<0.05)。结论长期饮用高氟水源会导致妇女体内的SHBG与E2水平受到影响。
Objective To study the effect of high fluoride water on the levels of sex hormone binding protein (SHBG) and estradiol (E2) in women. Methods According to the results of water quality survey, the fluoride content in water was accurately monitored. The survey sites were selected in 6 villages of a county, and 2 villages with normal drinking water, 2 villages with fluoride reduction and water diversion works, 2 villages with endemic fluorosis For the control group, water modification group and experimental group, adult women who lived in each village for at least 5 years or local growth were selected by cluster sampling method and included in each group. Demographic characteristics were surveyed by means of interview, Blood, morning urine and local drinking water, urine fluoride determination of fluoride content, SHBG and E2 levels, and compared. Results There was significant difference between the fluoride content in the three drinking water groups and the urinary fluoride content, serum SHBG and E2 levels in the three groups (P <0.05). The fluoride level in the experimental group was (2.45 ± 1.02) mg / (0.36 ± 0.14) mg / L in the control group (0.95 ± 0.32) mg / L in the control group (P <0.05) (1.44 ± 0.58) mg / L and 2.57 ± 0.96 mg / L, respectively. The urinary fluoride content in the water-modified group was lower than that in the experimental group (P <0.05). The serum SHBG in the experimental group was 41.62 ± 7.85) nmol / L, which was lower than that of the control group (57.20 ± 8.65) nmol / L (P <0.05). The serum E2 level in the experimental group was (52.28 ± 11.45) nmol / 13.54) nmol / L and (75.58 ± 14.35) nmol / L, respectively (P <0.05). There was a negative correlation between serum E2 level and age in the experimental group (P <0.05). CONCLUSIONS Long-term drinking of high-fluoride water causes the levels of SHBG and E2 in women to be affected.
