目的评价64排螺旋CT冠状动脉成像(CTA)在心肌桥伴冠心病诊断中的应用价值。方法与选择性冠脉造影(SCA)做比较,采用64排螺旋CT对101例疑诊冠心病患者的冠脉主干及主要分支进行重建和分析,评价其诊断心肌桥伴冠心病的灵敏度和特异度。结果CTA能够清晰显示冠脉主干及其分支的肌桥、狭窄、钙化,CTA诊断冠脉病变的灵敏度96.61%,特异度97.54%,阳性预测值98.12%,阴性预测值97.81%。其中对管腔狭窄>50%的病变灵敏度最高,达到100%。结论CTA对冠脉肌桥、狭窄病变均显影良好,对冠脉肌桥及冠心病诊断有较高的准确性,对钙化病变诊断率优于冠脉造影,可以作为疑诊冠心病人群无创性筛选检查手段。
Objective To evaluate the value of 64-slice spiral CT coronary angiography (CTA) in the diagnosis of coronary heart disease with myocardial bridge. Methods Compared with selective coronary angiography (SCA), 64 cases of spiral CT were used to reconstruct and analyze the main coronary artery and the main branches of 101 patients with suspected coronary heart disease. The sensitivity and specificity of the method for diagnosing myocardial bridge with coronary heart disease degree. Results CTA was able to clearly show the muscular bridge, stenosis and calcification of the main coronary artery and its branches. The sensitivity and specificity of CTA in the diagnosis of coronary artery disease were 96.61% and 97.54%. The positive predictive value was 98.12% and the negative predictive value was 97.81%. Among them, the sensitivity of lesions with> 50% lumen stenosis was the highest, reaching 100%. Conclusion CTA has a good visualization of coronary artery bridge and stenosis lesions, high accuracy of diagnosis of coronary artery bridge and coronary heart disease, diagnosis of calcification lesion is superior to coronary angiography, and can be used as a noninvasive Screening test means.
