苯妥英是一种常甩的抗癫痫和抗心律失常药物,它的有效治疗血浓度一般为10~20μg/ml,超过此范围时,会产生中毒现象。由于个体的差异性,相同剂量而血浓度可不一样。为了确保用药安全,宜在用药期间,经常检测血中苯妥英的含量进行监护。 苯妥英在碱性高锰酸钾溶液中被氧化成二苯酮后,用环已烷提取,在247nm处测出吸收值后进行运算。其具体操作步骤是:取病人血清1.0(或1.5)ml,置于2ml pH6.8的磷酸盐缓冲液中,用二氯甲烷10ml提取二次,合并提取液,加入7N NaOH7ml,摇匀,分离出碱液,加热除去残留的二氯甲烷,然后加饱和KMnO_4溶液2ml,摇匀密塞,于80℃水浴加热15~25分钟,取出冷至室温,加入2.5ml环己烷提取,在247nm处测出提取液吸收值y,苯妥英浓度t在1~30μg/ml血清时,与y的回归方程为y=0.02061x+0.003163,r=0.9999,回收率为98.32%±0.58%,cu%为0.59%。
