目的探讨螺旋CT与超声心动图在左心室功能评估中的对比。方法 2009年2月至2012年1月冠心病患者42例,所有患者均行超声心动图和螺旋CT检查,观察左心室功能。结果 42例均行超声心动图和螺旋CT检查,观察左心室功能收缩末期容积(ESV)、舒张末期容积(EDV)、射血分数(EF)每搏输出量(SV)并进行比较。两组相关指标比较差异无统计学意义(P>0.05)。结论 MSCT对心脏的数据分析建立在三维容积扫描基础上,与超声心动图的几何假设相比,更加接近心室腔的实际解剖形态,但是图像后处理需消耗较多时间,工作量大。而超声心动图没有放射性,无需使用对比剂,可以床旁操作,医疗费用低廉,所以在作为常规诊断方面具有MSCT不可取代的优势。多层螺旋CT检查对于心律的整齐有严格要求,心律不齐的患者不能得到准确的数据。再有,对于心率大于75次/min的患者进行扫描,冠状动脉造影图像清晰度欠佳,对心功能的准确性有一定的影响,必需降心率后进行检查。总之,多层螺旋CT与超声心动图在左心室功能评估中,MDCT扫描定位准确,功能评估十分准确,可以反复多次测量,主观影响较小,有着自己的独特的优势。
Objective To investigate the contrast between spiral CT and echocardiography in the assessment of left ventricular function. Methods From February 2009 to January 2012, 42 patients with coronary heart disease were enrolled. All patients underwent echocardiography and spiral CT examination to observe left ventricular function. Results Eighteen patients underwent echocardiography and spiral CT. The left ventricular end-systolic volume (ESV), end-diastolic volume (EDV) and ejection fraction (EF) were measured and compared. There was no significant difference between the two groups (P> 0.05). Conclusions MSCT data analysis of the heart is based on three-dimensional volume scanning. Compared with the geometric assumptions of echocardiography, MSCT is closer to the actual anatomy of the ventricular cavity. However, post-processing of images requires more time and requires more work. Echocardiography is radioactive, does not require the use of contrast media, can be bedside, and has low medical costs. Therefore, MSCT has an irreplaceable advantage in routine diagnosis. Multi-slice spiral CT for the rhythm of the tidy strict requirements, arrhythmia patients can not get accurate data. In addition, for patients with heart rate greater than 75 beats / min, coronary angiography has poor image clarity and has some impact on the accuracy of cardiac function, and must be checked after lowering the heart rate. In short, MSCT and echocardiography in the assessment of left ventricular function, MDCT scan accurate positioning, function evaluation is very accurate, can be repeated multiple measurements, the subjective impact of small, has its own unique advantages.
