对99例可疑肝占位病变患者行肝动脉灌流造影(IHA)与碘油造影(Lp-HA),确诊肝癌94例,IHA阳性率97.9%,Lp-HA均阳性。对非主瘤所在肝叶病变的检出率,IHA为83.3%(35/42),Lp-HA为100%。Lp-HA能发现IHA漏诊的2例左叶小肝癌及7例非主瘤所在肝叶的小癌结节,且能携带混合的抗癌剂长期滞留于癌组织起到持续的抗癌作用。作者建议对疑诊肝癌病人行普通肝动脉造影时均宜同时作Lp-HA。Lp-HA的肝癌聚碘象可分絮状、斑块状、池状、弥漫颗粒状4种。一例Lp—HA后2月手术切除标本的癌灶与聚碘影象一致,说明聚碘影象可以作为肝癌大体形态分型诊断的依据。
99 cases of suspicious liver lesions were treated with hepatic arterial angiography (IHA) and lipiodol angiography (Lp-HA). 94 cases of liver cancer were diagnosed. The positive rate of IHA was 97.9% and Lp-HA was positive. The detection rate of liver lesions in non-host tumors was 83.3% (35/42) for IHA and 100% for Lp-HA. Lp-HA could find two cases of left hepatocellular carcinoma and seven cases of hepatocellular nodules that were not diagnosed by IHA and could carry a mixed anticancer agent for a long period of time to remain in the cancer tissue and play a sustained anticancer effect. The authors suggest that Lp-HA should be performed simultaneously for common hepatic arteriography in patients with suspected liver cancer. Lp-HA’s hepatoma polyiodine can be divided into floccular, plaque, pool, and diffuse granular. One case of Lp-HA after surgical resection of the lesion in 2 months consistent with the image of iodine, indicating that the iodine image can be used as a basis for the diagnosis of hepatocellular carcinoma.
