目的 :为经蝶进路手术提供影像解剖学资料。方法 :对 5 0具成人头颅标本行冠状位、矢状位及水平位薄层CT扫描。观测鞍区手术最常用的 2种进路经鼻外筛窦蝶窦进路及经鼻中隔蝶窦进路的相关影像解剖。结果 :前鼻棘至蝶窦前壁的平均距离 :左侧 :5 6 .5± 5 .9mm ;右侧 :5 4.9± 7.2mm ;鼻根点至蝶窦前壁的平均距离 :左侧 :44 .3± 5 .6mm ;右侧 :44 .0± 6 .5mm ;前鼻棘至鞍底中点的平均距离 :左侧 :74.5± 6 .4mm ;右侧 :73 .8± 6 .8mm ;鼻根点至鞍底中点的平均距离 :左侧 :6 0 .9± 7.2mm ;右侧 :6 0 .2± 7.8mm。 2种进路的解剖距离相差 12~ 14mm。结论 :CT影像解剖测量对选择不同的手术进路更具实际的指导意义。
Objective: To provide image anatomy data for transsphenoidal approach surgery. Methods: Fifty adult skulls underwent coronal, sagittal and horizontal thin layer CT scans. Observing the most commonly used 2 methods of transscleral operation through the nasal septum sinus sphenoid sinus approach and transnasal septal sphenoid sinus imaging related anatomy. RESULTS: The average distance from the anterior nasal spine to the anterior sphenoidal wall was 56.5 ± 5.9 mm on the left and 4.9 ± 7.2 mm on the right: the average distance from the nasal root to the anterior sphenoid: left: 44.3 ± 5.6mm; right: 44.0 ± 6.5mm; average distance from anterior nasal spine to midpoint of saddle: left: 74.5 ± 6 .4mm; right: 73.8 ± 6.8mm ; The average distance from the nasal point to the mid-point of the saddle: left: 60.9 ± 7.2 mm; right: 60.2 ± 7.8 mm. The anatomical distances of the two approaches differ by 12-14 mm. Conclusion: CT image anatomical measurement of the choice of different surgical approach more practical guidance.
