目的观察纳洛酮治疗急性有机磷中毒昏迷的疗效。方法选择急性有机磷中毒昏迷78例,随机分为观察组40例及对照组38例。入院后均立即予彻底清除残存毒物,给予乙酰胆碱拮抗剂、胆碱酯酶复能剂及常规对症治疗。观察组在此基础上加用纳洛酮治疗:首剂1.2mg静脉注射,之后0.4mg/h静脉滴注,直至患者神志转清醒,生命体征平稳。结果两组对比,观察组治疗后胆碱酯酶活力较对照组明显提高,阿托品化时间和清醒时间明显缩短,其差异具有统计学意义(P<0.01)。结论急性有机磷中毒昏迷患者在常规治疗的基础上加用纳洛酮治疗具有明显的治疗效果,未发现不良反应,是较为理想的辅助用药,适宜临床应用。
Objective To observe the efficacy of naloxone in the treatment of acute organophosphate poisoning. Methods 78 patients with acute organophosphate poisoning were randomly divided into observation group (40 cases) and control group (38 cases). Immediately after admission to remove the residual poisons, given acetylcholine antagonist, cholinesterase agents and conventional symptomatic treatment. The observation group added naloxone on this basis: the first dose of 1.2mg intravenously, followed by 0.4mg / h intravenous infusion until the patient consciousness to wake up, vital signs stable. Results Compared the two groups, the activity of cholinesterase in the observation group was significantly higher than that in the control group, and the time of atropineization and awake time were significantly shortened. The difference was statistically significant (P <0.01). Conclusion Acute organophosphate poisoning coma patients with naloxone on the basis of conventional treatment has obvious therapeutic effect, no adverse reactions, is an ideal auxiliary drug, suitable for clinical application.
