目的:探讨侧脑室外引流并血肿微创清除术结合腰穿脑脊液置换治疗重型小脑出血的疗效。方法:分别统计30例经微创手术治疗的重型小脑出血患者术前APACHEII评分、预测病死率及30天实际病死率,采用二项分布检验统计方法比较实际病死率与预测病死率之间的差异。结果:根据APACHEII评分算出术前平均预测病死率为61.1%,30天实际病死率为26.7%,两者有统计学差异(P<0.01)。结论:侧脑室外引流并血肿微创清除术结合腰穿脑脊液置换能显著改善重型小脑出血预后,降低病死率。
Objective: To investigate the curative effect of lateral ventricle drainage and minimally invasive hematoma dissection combined with lumbar puncture and cerebrospinal fluid replacement for treatment of severe cerebellar hemorrhage. Methods: The preoperative APACHEII score, predicted mortality and 30-day actual mortality of 30 cases of minimally invasive treatment of patients with cerebellar hemorrhage were collected. The binomial distribution test was used to compare the difference between the actual mortality and the predicted mortality . Results: According to the APACHEII score, the average preoperative mortality was 61.1% and the actual mortality at 30 days was 26.7%. There was a significant difference between the two (P <0.01). Conclusion: Extracerebral drainage of the lateral ventricle combined with minimally invasive hematoma dissection combined with lumbar puncture and cerebrospinal fluid replacement can significantly improve the prognosis and reduce the mortality of severe cerebellar hemorrhage.
