目的 探讨直肠癌及其近旁粘膜组织中维生素D受体 (VDR)的表达。方法 采用免疫组化结合显微光度测量图像分析技术 ,对 2 1例直肠癌及其近旁粘膜中VDR表达与正常大肠粘膜比较。结果 直肠癌组织中VDR表达 ( 2 4± 0 7)显著低于正常大肠粘膜 ;癌旁 2cm粘膜中VDR表达 ( 4 6± 0 5 )介于癌组织与正常粘膜 ( 6 0± 0 6)之间 ,与 2者差异均有显著意义。癌组织中VDR的表达在不同的年龄、性别、Dukes分期、组织学分型、肿瘤生长方式、浸润深度之间差异无显著意义 ;VDR表达与癌组织分化程度之间有显著联系 ,高、中、低分化 3组VDR表达依次降低且差异有显著意义 (P <0 0 5 )。其中癌旁 2cm粘膜DNA异倍体者占 5 7 1% ,增殖指数显著高于正常粘膜。结论 直肠癌及其近旁粘膜组织中VDR表达下降。癌旁 2cm粘膜具有向肿瘤转化的倾向。
Objective To investigate the expression of vitamin D receptor (VDR) in rectal cancer and its adjacent mucosa. Methods Immunohistochemistry combined with microphotometric image analysis technique was used to compare the expression of VDR in 21 cases of rectal cancer and its adjacent mucosa with normal colorectal mucosa. Results The expression of VDR in rectal cancer tissues was significantly lower than that in normal colorectal mucosa. The expression of VDR in the adjacent 2cm mucosa was (46±0.5) between cancer tissues and normal mucosa (60±0.6). There were significant differences between the two and the two. There was no significant difference in the expression of VDR in cancer tissues at different ages, genders, Dukes stages, histological types, tumor growth patterns, and depth of invasion. There was a significant relationship between VDR expression and the degree of differentiation of cancer tissues. The expression of VDR in the poorly differentiated group 3 decreased in turn and the difference was significant (P < 0 05). Among them, 57% of mucosal DNA aneuploids were located adjacent to the tumor and the proliferation index was significantly higher than that of the normal mucosa. Conclusion The expression of VDR in rectal cancer and its adjacent mucosa decreases. The 2 cm mucosa adjacent to the cancer has a tendency to transform into tumors.
