目的探讨MRI磁敏感加权成像(SWI)在诊断弥漫性轴突损伤微出血的临床价值。方法 40例临床确诊弥漫性轴突损伤微出血患者均行MRI平扫和磁敏感加权成像序列、常规MRIT_2WI、T_2FLAIR、磁共振扩散加权成像(DWI)及CT扫描。结果 MRI平扫T_2WI见13例表现点状高信号影,病灶检出率为32.5%;T_2FLAIR上发现病变16例,病灶检出率为40%;DWI上发现异常高信号影28例,病灶检出率为70%;CT见10例表现为点状高密度出血灶,病灶检出率为25%。40例患者均发现异常小出血灶影,病灶检出率为100%,且发现病变的数目及范围明显增多、增大。结论 MRI磁敏感加权成像序列诊断弥漫性轴突损伤微出血较CT和常规MRI检查序列有明显优势。
Objective To investigate the clinical value of MRI magnetic resonance weighted imaging (SWI) in the diagnosis of diffuse axonal injury micro-hemorrhage. Methods Totally 40 patients with diffuse axonal injury and micro-hemorrhage were examined by MR plain imaging and magnetic susceptibility weighted imaging sequence, routine MRIT_2WI, T_2FLAIR, diffusion weighted imaging (DWI) and CT scan. Results Thirteen cases showed high punctate signal intensity on MRI scan, with the detection rate of 32.5%. Sixteen lesions were detected on T_2FLAIR and the detection rate was 40%. On the DWI, there were 28 cases of abnormal high signal intensity and focal lesion Out rate of 70%; CT see 10 cases showed punctate high-density bleeding, the detection rate was 25% lesions. 40 patients were found to have abnormal small bleeding lesions, the detection rate was 100% lesions, and found that the number and scope of lesions increased significantly increased. Conclusion MR magnetic resonance weighted imaging sequence diagnosis of diffuse axonal injury than CT and conventional MRI examination sequence has obvious advantages.
