患者,女性,26岁,2004年6月28日上午9时许口服25%黑杀乳油约150ml,恶心、呕吐2次胃内容物。10时20分开始神志不清,呼吸浅弱,并伴肌肉颤动,无大小便失禁,当地村卫生室给予阿托品10 mg 肌注,11时送入我院。入院时查体:T 不升,BP 130/70mm Hg,HR 84次/min,R 8次/min,神志不清,深昏迷,双瞳约针尖大小,光反射迟钝,面色苍白,双肺满布湿性罗音,心音低钝,节律不齐,频发早博。诊断:黑杀乳油中毒。处理:立即清水彻底洗胃,20%甘露醇250 ml 导泻,吸氧。急查胆碱脂酶,及时给予尼可刹米2.25+10%葡萄糖500ml 静滴
