目的系统评价正念减压(MBSR)对缓解艾滋病病毒(HIV)感染者/艾滋病(AIDS)病人(简称HIV/AIDS病人)压力、抑郁等心理问题,以及提高CD4~+T淋巴细胞(简称CD4细胞)水平的效果。方法通过检索PubMed、Cochrane Library、CBM、CNKI等中外文数据库,收集建库至2014年10月关于MBSR应用于HIV/AIDS病人的临床试验,按照统一标准完成文献检索、筛选、资料提取及质量评价工作。采用RevMan 5.2软件进行Meta分析。结果最终纳入5个研究,共448例HIV/AIDS病人。Meta分析结果显示:MBSR干预组心理压力得分[标准化均数差(SMD)=-0.27;95%可信区间(CI):-0.58~-0.05)],抑郁水平得分(SMD=-0.28;95%CI:-0.49~-0.08),均低于对照组,差异有统计学意义(P=0.01,P=0.007)。MBSR组CD4细胞计数高于对照组,差异有统计学意义(SMD=1.47;95%CI:0.14~2.79,P=0.03)。MBSR组在焦虑及缓解HIV/AIDS病人功能评估等指标上与对照组的干预效果相同。结论 MBSR能够缓解HIV/AIDS病人的心理压力及抑郁情绪,并能提高其CD4细胞水平,但不能改善焦虑水平及身体功能等。
Objective To systematically evaluate the effect of mind-bending decompression (MBSR) on alleviating psychological problems such as stress and depression in HIV / AIDS patients and increasing CD4 + T lymphocytes (CD4 cells ) Horizontal effect. Methods The databases of PubMed, Cochrane Library, CBM, CNKI and other Chinese and foreign language databases were searched and collected. The clinical trials of MBSR in HIV / AIDS patients were collected from October 2014 to October 2014, and the literature search, screening, data extraction and quality evaluation were completed according to uniform standards jobs. Meta-analysis was performed using RevMan 5.2 software. Results Finally included in 5 studies, a total of 448 cases of HIV / AIDS patients. The results of Meta-analysis showed that the score of psychological stress (mean standard deviation (SMD) = -0.27; 95% CI: -0.58 ~ -0.05), depression level (SMD = -0.28; 95 % CI: -0.49 ~ -0.08) were lower than the control group, the difference was statistically significant (P = 0.01, P = 0.007). The CD4 cell count in MBSR group was significantly higher than that in control group (SMD = 1.47; 95% CI: 0.14-2.79, P = 0.03). The MBSR group had the same effect as the control group on anxiety and remission of HIV / AIDS patients. Conclusions MBSR can alleviate the psychological stress and depression in HIV / AIDS patients and increase the level of CD4 cells, but can not improve the anxiety level and body function.
