糖尿病心肌病(DCM)是一类独立的特异性心肌病,可诱发心力衰竭、心律失常、心源性休克和猝死,已成为糖尿病患者最主要的死因之一。DCM发病机制复杂,涉及心肌细胞代谢障碍、心肌微血管病变、自主神经病变、胰岛素抵抗及细胞因子异常等多个方面。而脂联素(APN)因其具有降血糖、扩张血管、抗凋亡、抗炎、抗氧化作用以及能缓解血管内皮功能障碍和高血压,也能防止动脉粥样硬化、心肌梗死和DCM,故被认为是一种糖尿病心脏病的保护因子。该文就APN在DCM发生发展中所起作用的研究进展予以综述。
Diabetic cardiomyopathy (DCM) is an independent class of cardiomyopathy that can induce heart failure, arrhythmia, cardiogenic shock, and sudden death, and has become one of the leading causes of death in people with diabetes. The pathogenesis of DCM is complex, involving many aspects of myocardial cell metabolism, myocardial microangiopathy, autonomic neuropathy, insulin resistance and cytokine abnormalities. Adiponectin (APN) can prevent atherosclerosis, myocardial infarction and DCM because of its hypoglycemic, dilated blood vessels, anti-apoptotic, anti-inflammatory, anti-oxidant effects and can relieve vascular endothelial dysfunction and hypertension. It is considered a protective factor of diabetic heart disease. This article reviews the research progress of APN in the development of DCM.
