目的采用应变、应变率成像(S/SRI)技术评价阵发性房颤(PAF)患者的左心房(LA)功能。方法 57例PAF患者根据LA有无扩大分为2组:PAF1组(LA大小正常)32例和PAF2组(LA扩大)25例,另设29例健康志愿者为对照组。分析三组被检者的LA S/SR,比较各时相的S、SR峰值大小和运动同步性。结果 (1)心房泵血期和储血期的S、SR在对照组、PAF1组和PAF2组之间依次降低(均P<0.05);PAF1组和PAF2组的通道期S、SR也较对照组减低(均P<0.05)。(2)PAF1组和PAF2组泵血期的运动同步性均较对照组差(均P<0.05);PAF2组储血期的运动同步性也较对照组差(P<0.05)。结论 PAF患者LA功能在LA大小正常时已有受损,LA扩大时进一步下降。S/SRI技术可以在早期敏感地检测出LA机械功能的异常改变。
Objective To evaluate the left atrium (LA) function in patients with paroxysmal atrial fibrillation (PAF) by using strain and strain rate imaging (S / SRI) technique. Methods Fifty-seven patients with PAF were divided into two groups according to the presence or absence of LA: 32 patients in the PAF1 group (normal LA size) and 25 patients in the PAF2 group (extended LA), and 29 healthy volunteers as the control group. Three groups of subjects were analyzed LA S / SR, comparing the S, SR peak size and motion synchronization of each phase. Results (1) The S and SR of aortic pump and blood storage were decreased in turn between the control group, PAF1 group and PAF2 group (all P <0.05), while the S, SR of the PAF1 group and the PAF2 group were also lower than those of the control group Group decreased (all P <0.05). (2) The synchronicity of pumping blood in PAF1 and PAF2 groups was worse than that in control group (all P <0.05). The synchronicity in PAF2 group was also lower than that in control group (P <0.05). Conclusions LA function in patients with PAF has been impaired at normal LA size and further decreased when LA is expanded. The S / SRI technique can detect anomalous changes of LA mechanical function in the early stage of sensitivity.
