Objective To raise awareness of adult chronic active Epstein-Barr virus (CAEBV) infection. Methods The clinical, laboratory characteristics, treatment response and outcome of 3 patients diagnosed as CAEBV infection were retrospectively analyzed. Results The clinical manifestations and EB virological examination of 3 patients were consistent with the clinical features of CAEBV infection. Two of three patients died and the other one lost their follow-up. Conclusions CAEBV infection is a rare and often fatal disease with poor prognosis. For patients with CAEBV infection, active measures should be taken early and immune reconstitution should be performed to completely eliminate the lymphocytes infected with virus or cloned.
