目的 :分析心功能不全患者限制性充盈 (RF)和非限制性充盈 (NRF)时二尖瓣血流频谱 (MFP)。方法 :选择 48例左室收缩功能不全 (VLEF <0 45 )的陈旧性心肌梗塞患者和 2 8名健康对照为研究对象。采用经胸多普勒超声心动图 (TDE)检测MFP。结果 :按照二尖瓣舒张早期最大血流速度 (E)与舒张晚期最大血流速度 (A)比值 (E/A)和E波减速时间 (DT)将病例组分为RF组和NRF组。和对照组比较 ,NRF组E和E/A减小 (P均 <0 0 1) ,左室舒末内经 (LVD)轻度扩大〔5 1.3± 5 .68)mm ,P <0 .0 5〕 ;RF组E、E/A增大 (P <0 0 5~ 0 0 0 1) ,DT缩短 (P <0 0 5 ) ,LA( 4 1 61± 7 5 6)mm和LVD( 63 5± 10 2 9)mm显著扩大 (P均 <0 0 0 1)。RF组与NRF组比较 ,LVEF减低 6%(P <0 0 5 ) ,心功能分级Ⅲ~Ⅳ (NYHA)患者所占比例较大 (P <0 0 5 )。结论 :NRF组和RF组患者的MFP参数均有不同程度的异常。较之NRF组 ,RF组MFP有显著异常。
PURPOSE: To analyze mitral flow spectrum (MFP) in patients with cardiac insufficiency and without restrictive filling (NRF). Methods: Forty-eight patients with left ventricular systolic dysfunction (VLEF <455) and 28 healthy controls were enrolled in this study. The transthoracic Doppler echocardiography (TDE) was used to test the MFP. Results: The cases were divided into RF group and NRF group according to the ratio of the maximum diastolic velocity of early mitral diastole (E) to the maximum diastolic velocity of diastolic phase (A) (E / A) and the decelerating time of E wave (DT) Compared with the control group, the E and E / A of NRF group decreased (P <0 01), LVD slightly enlarged 〔5 1.3 ± 5.68 mm], P0.05 (P <0.05-0.01), DT shortened (P <0.05), LA (4161 ± 765) mm and LVD ± 10 2 9) mm significantly (all P <0 0 0 1). Compared with NRF group, LVEF was decreased by 6% (P <0.05) in RF group and was significantly higher in NYHA class (P <0.05). Conclusion: The MFP parameters of patients in NRF group and RF group all have different degrees of abnormalities. RF Group MFPs were significantly more abnormal than the NRF group.
