[目的]探讨不同程度肺功能损害的慢性阻塞性肺疾病(COPD)急性加重与大气质量、气温等环境因素的关系。[方法]收集2004年1月1日—2009年7月31日,上海一所中心医院收住入院的慢性阻塞性肺疾病急性加重的病例,按入院后肺功能检查结果分成轻度、中度、重度3组。分析每组每日入院人数与入院当天(d0)、前1天(d-1)、前3天(d-3)、前6天(d-6)的大气污染物浓度、气温之间的相关性。[结果]肺功能重度损害COPD组每日入院人数与大气质量因素均呈现一定的正相关。与最高、最低气温呈一定的负相关。肺功能中度损害COPD组每日入院人数与入院前3天PM10的API值呈正相关,与最低气温呈负相关。肺功能轻度损害COPD组每日入院人数与入院当天的SO2的API值呈正相关,与最低气温呈负相关。[结论]慢性阻塞性肺疾病急性加重的发生与大气质量、气温存在一定的相关性,尤其是对于肺功能重度损害的COPD病人。提示大气污染、气温降低是慢性阻塞性肺病急性加重的重要诱因。
[Objective] To investigate the relationship between acute exacerbation of chronic obstructive pulmonary disease (COPD) and environmental factors such as air quality and air temperature with varying degrees of pulmonary dysfunction. [Methods] From January 1, 2004 to July 31, 2009, a case of acute exacerbation of chronic obstructive pulmonary disease admitted to a central hospital in Shanghai was divided into mild and moderate according to the results of pulmonary function tests after admission , Severe 3 groups. The daily intake of air pollutants in each group was compared with the air pollutant concentration at the day before admission (d0), before the first day (d-1), the first three days (d-3), the first six days Correlation. [Results] There was a positive correlation between the number of daily hospital admission and the air quality in patients with severe pulmonary function impairment. And the highest and lowest temperature showed a certain negative correlation. Moderately impaired lung function The number of daily admission to the COPD group was positively correlated with the PM10 API value at 3 days before admission and negatively correlated with the lowest temperature. Mild pulmonary function impairment The daily number of hospital admission in COPD group was positively correlated with the API value of SO2 on the day of admission, and negatively correlated with the lowest temperature. [Conclusion] The occurrence of acute exacerbation of chronic obstructive pulmonary disease is related to the air quality and air temperature, especially for COPD patients with severe pulmonary function impairment. Prompted air pollution, temperature reduction is an important cause of acute exacerbations of chronic obstructive pulmonary disease.
