目的探讨负压封闭引流术在手部热压伤中的疗效。方法对15例手部热压伤患者彻底清创后,切痂,应用负压封闭引流材料覆盖创面,持续负压吸引,维持负压在150~280 mmHg,5~7 d后除去负压封闭引流材料,行自体中厚皮移植术。结果 15例患者创面愈合时间19~32 d,平均24.6 d。其中14例患者中厚皮片全部成活;1例患者植皮术后,部分皮片坏死,再次行局部皮瓣转移术后创面愈合。随访6~12个月,手功能恢复满意。结论应用负压封闭引流术治疗手部热压伤能促进创面肉芽组织生长,使手术简单化,减轻患者痛苦。
Objective To investigate the curative effect of negative pressure closed drainage in hand thermal compression. Methods Fifteen cases of hand-crushed patients were completely debrided and then escharectomy. The wounds were covered with negative pressure drainage material and suctioned continuously. The negative pressure was maintained at 150-280 mmHg. After 5-7 days, negative pressure was removed Drainage materials, line autologous medium thickness skin graft. Results The wound healing time of 15 patients was 19 ~ 32 days with an average of 24.6 days. Among them, all the 14 cases were alive. One patient underwent skin grafting and some of the skin was necrotic. The wounds were healed again after local flap transfer. Follow-up 6 to 12 months, hand function restored satisfactory. Conclusion The application of negative pressure closed drainage in the treatment of hand hot crushing can promote the growth of wound granulation tissue, simplify the operation and relieve the suffering of patients.
