[目的]了解20世纪70年代以来河北省居民肺癌死亡情况,为防治工作提供依据。[方法]对河北省20世纪70年代(1973~1975年)、80年代(1983~1985年)、90年代(1990~1992年)、21世纪初(2004~2005年)居民死亡原因调查资料进行分析。[结果]2004~2005年在河北省抽取有代表性的18个市县,抽样市县共13791868人(男性7089291人,女性6702577人),总死亡人数为82878人(男性48611人,女性34267人),粗死亡率为600.92/10万(男性为685.70/10万,女性为511.25/10万),其中死于肺癌的有4132人(男性2735人,女性1397人),粗死亡率为29.96/10万(男性为38.58/10万,女性为20.84/10万),标化死亡率男性为40.29/10万,女性为20.60/10万)。21世纪初与90年代的标化死亡率相比,男性上升了35.20%,女性上升了35.01%,与上世纪70、80和90年代比较呈明显上升趋势(男性χ2=3451.80,P﹤0.001;女性χ2=1626.54,P﹤0.001)。肺癌死亡率的最低年龄组为15岁~组,最高年龄组为85岁~组,中位死亡年龄为68.0岁。男性死亡率高于女性,性别比为1.85︰1。地理分布是张家口死亡率最高,武安最低。城乡分布是城市显著高于农村(男性χ2=190.29,P﹤0.001,女性χ2=92.04,P﹤0.001)。[结论]2004~2005年河北省居民肺癌死亡率有明显的上升趋势。
[Objective] To understand the death of lung cancer among residents in Hebei Province since the 1970s and provide the basis for prevention and treatment. [Method] The survey data of the cause of death of residents in Hebei Province in the 1970s (1973 ~ 1975), 1980s (1983 ~ 1985), 1990s (1990 ~ 1992) and early 21st century (2004 ~ 2005) analysis. [Results] A total of 13791868 people (7089291 males and 6702577 females) were sampled in Hebei Province from 2004 to 2005. The total number of deaths was 82878 (48611 males and 34267 females) ), With a crude death rate of 600.92 / 100,000 (685.70 / 100,000 for males and 511.25 / 100,000 for females), of which 4132 died of lung cancer (2735 males and 1397 females) with a crude mortality rate of 29.96 / 100,000 (male 38.58 / 100000, female 20.84 / 100000), standardized mortality of 40.29 / 100000 males and 20.60 / 100000 females). Compared with the normalized mortality rate in the early 21 st century and the 90 ’s, men increased by 35.20% and women increased by 35.01%, showing an obvious upward trend compared with the 1970s, 1980s and 1990s (male χ2 = 3451.80, P <0.001; Female χ2 = 1626.54, P <0.001). The lowest age group for lung cancer was 15 years of age, the highest age group was 85 years of age, and the median age of death was 68.0 years. Male mortality is higher than that of women, with a sex ratio of 1.85: 1. Zhangjiakou is the highest geographical distribution of death rates, the lowest in Wu’an. The urban-rural distribution was significantly higher in cities than in rural areas (χ2 = 190.29 for men, P <0.001, χ2 = 92.04 for women, P <0.001). [Conclusion] The mortality rate of lung cancer in Hebei Province from 2004 to 2005 increased obviously.
