目的 :探讨Commoncavity畸形患者人工耳蜗植入后的语言康复效果。方法 :对 6例Commoncavity畸形患者采用经乳突外半规管进路行人工耳蜗植入 ,并与 6例耳蜗发育正常组进行听力情况比较。结果 :32个电极全部植入者 3例 ,2 8个电极植入者 1例 ,2 6个电极植入者 1例 ,2 2个电极植入者 1例。与正常组比较 ,术中术后并发症差异无显著性意义。术中脑脊液多量流出者 2例 ,少量流出 3例 ,无流出 1例。两组术后听力接近 ,均在 30~ 4 0dBHL左右 ,无严重并发症发生 ,大多数患者术后听觉语言康复效果与正常耳蜗者接近 ,少数稍差。长期效果有待进一步观察。结论 :人工耳蜗适用于Commoncavity畸形患者 ,但手术前应进行全面的听力学及影像学评估。
Objective: To investigate the effect of language rehabilitation after cochlear implantation in Commoncavity patients. Methods: 6 cases of Commoncavity malformations were treated with cochlear implants through the semicircular canal of mastoid, and compared with 6 cases of normal cochlear development group. Results: All 32 electrodes were implanted in 3 cases, 28 cases were implanted in 1 case, 2 cases were implanted in 1 case and 2 cases were implanted in 1 case. Compared with the normal group, intraoperative and postoperative complications did not differ significantly. There were 2 cases of cerebrospinal fluid effusion, 3 cases of small amount of effusion and 1 case of no effusion. The hearing of the two groups was close to 30 ~ 40dBHL, with no serious complication. Most patients had auditory language rehabilitation effect close to that of normal cochlea. Long-term results need to be further observed. CONCLUSIONS: Cochlear implants are suitable for Commoncavity patients, but comprehensive audiology and radiological assessment should be performed prior to surgery.
