目的 探讨提高大肠癌淋巴结微转移诊断率的敏感方法。方法 用逆转录聚合酶链式反应 (RT -PCR)检测基质金属蛋白酶 - 7(MMP - 7)mRNA在 38例大肠癌和 2 5个淋巴结中的表达 ,对部分淋巴结再次行组织学检查。结果 97 4% (37/ 38)的大肠癌中MMP - 7mRNA表达阳性 ,而正常粘膜几乎不表达。传统组织学检查发现癌转移的 13个淋巴结MMP - 7mRNA表达均为阳性 ;未见转移的 12个淋巴结MMP - 7mRNA表达6个阴性、6个阳性 ,再次行组织学检查于 6个MMP - 7阳性者中发现 3个有微转移。结论 RT -PCR方法检测淋巴结中MMP - 7mRNA的表达是诊断大肠癌淋巴结微转移的敏感方法 ,可以作为传统组织学检查的补充。
Objective To explore a sensitive method to improve the diagnostic rate of lymph node micrometastasis in colorectal cancer. Methods Reverse transcription polymerase chain reaction (RT-PCR) was used to detect the expression of matrix metalloproteinase-7 (MMP-7) mRNA in 38 cases of colorectal cancer and 25 lymph nodes. The histological examination of some lymph nodes was performed again. RESULTS: MMP-7 mRNA was positive in 97% (37/38) of colorectal cancers, but almost normal mucosa was not expressed. Traditional histological examination revealed positive expression of MMP-7 mRNA in 13 lymph nodes metastasized to cancer; MMP-7 mRNA expression in 12 lymph nodes without metastasis was negative in 6 and 6 positive, and histological examination in 6 MMP-7 positive was performed again. Among them, 3 were found to have micrometastases. Conclusion The detection of MMP-7 mRNA expression in lymph nodes by RT-PCR is a sensitive method for diagnosing lymph node micrometastasis in colorectal cancer and can be used as a supplement to traditional histological examination.
